Evaluation Questionnaire
I've put together the below
questionnaire as a free tool for non-profit
organizations to help them look at the big picture of
their organization and to help them possibly spot areas
where they might need to focus some work. This
also can be a tool for me to identify the best ways to
help your organization, if you choose to share it with
me. There are 2 versions below - a pdf (portable
data format such as Acrobat) file version that you can
print out to write on by hand, and also a rtf (rich text
format) file version that you can open in your word
processor and into which you can type directly.
